INSURANCE
At Excel OSPT we provide our patients the courtesy of verifying their Physical Therapy benefits. However, it is always in the patient's best interest to be familiar with their Physical Therapy benefits package. We accept all major insurance plans, Medicare, Workers Compensation and No Fault. For questions please contact us, we would be happy to walk you through the process. 

​No prescription? No problem. The ability to visit an orthopedic physical therapist without a physician’s prescription or referral is called Direct Access, and it can get you on a speedier path to recovery.

WHAT TO WEAR TO THERAPY

When it comes to choosing an appropriate outfit for your physical therapy appointments there are a few basic things to keep in mind. The clothing you wear may have to do with the specific injury or body part that is being treated during your therapy appointment. Although some of these may differ based on the body part you’re focusing on — there are generally a few considerations to follow.

Consider these three factors:

1.    What is your range-of-motion like in the clothing? Are you able to move around easily?

2.    Can your doctor or physical therapist easily access or see the affected areas?

3.    How comfortable are you in the clothing you’ve decided on? Will you be able to do exercises in them? Can you move around or stretch comfortably?

It is especially important that you avoid any tight-fitting, restrictive clothing. Proper physical therapy attire includes:

•    Loose t-shirts

•    Athletic shorts  

•    Workout pants/training pants — pants with a loose or elastic waistband.

•    Proper undergarments — sports bras, etc.

•    Proper foot ware

Always wear socks and sneakers to your physical therapy appointment — unless you are unable to. Supportive athletic sneakers, or tennis shoes, are your best options. Stay away from wearing sandals, heels, or slip-on shoes (unless absolutely necessary). Slip-on shoes, open-toed shoes, and shoes that do not have non-skid soles can put you at risk for an injury.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED OR DISCLOSED AND HOW YOU CAN GET ACCESS TO INFORMATION. PLEASE REVIEW IT CAREFULLY. 

 

EXCEL PHYSICAL THERAPY’S LEGAL DUTY

Excel Physical Therapy is required is required by law to protect the privacy of your personal health information, provide this notice about our information practices,  and follow the information practices that are described herein. 

 

USES AND DISCLOSURES OF HEALTH INFORMATION

Excel Physical Therapy uses your personal health information primarily for treatment; obtaining payment for treatment; conducting internal administrative activities and evaluating the quality of care that we provide. For example, Excel Physical Therapy may use your personal health information to contact you to provide appointment reminders, or information about treatment alternatives or other health related benefits that could be of interest to you. Excel Physical Therapy may also use or disclose your personal health information without prior authorization for public health purposes, for auditing purposes, and for emergencies. We also provide information when required by law. In any other situation, Excel Physical Therapy’s policy is to obtain your written authorization before disclosing your personal health information. If you provide us with a written authorization to release your information for any reason, you may later revoke that authorization to stop further disclosures at any time. Excel Physical Therapy may change its policy at any time. When changes are made, a new Notice of Information will be provided to you. You may also request an updated copy of our Notice of Information Practices at any time. 

 

PATIENT'S INDIVIDUAL RIGHTS

You have the right to review or obtain a copy of your personal health information at any time. You have the right to request that we correct any inaccurate or incomplete information in your records. You also have the right to request a list of instances where we have disclosed your personal health information for reasons other than treatment, payment or other related administrative purposes. You may also request in writing that we not use or disclose your personal health information for treatment, payment and administrative purposes except when specifically authorized by you, when required by law or in emergency circumstances. Excel Physical Therapy will consider all such requests on a case-by-case basis, but the practice is not legally required to accept them. 

 

CONCERNS AND COMPLAINTS

If you are concerned that Excel Physical Therapy may have violated your privacy rights or if you disagree with any decisions we have made regarding access or disclosure of your personal health information, please contact our owner at the address listed below. You may also send a written complaint to the U.S. Department of Health and Human Services.
